This is a four around one pick-and-roll play that the Phoenix Suns used to run for Steve Nash. It can be a side pick-and-roll play or a middle pick-and-roll play depending on how the point guard's defender is guarding the point guard.

Frame 1
If #1's defender forces to the middle, #5 will set the screen so that he can attack the middle. #4's man will space away to the help-side wing. After #1 clears the ball screen, #5 rolls to the basket.
#1 can attack the basket, pull up for a three-point shot if his man goes under the ball screen, hit #5 rolling to the basket or kick the ball out to any of the perimeter players if their man comes to help stop the drive.
Frame 2
If #1's defender takes away the middle, #5 will set the screen so that #1 can attack along the sideline. When #4 sees this, he will come back to the help-side slot and look to receive the basketball. #5 will roll to the basket once #1 clears the ball screen.
