The LA Clippers ran this lob play early in game 5 against the Utah Jazz. As 1 gets the dribble handoff from 5, 4 runs off of a double screen as a decoy clearing the defense to set up the lob. 2 sets a back screen for 5 putting the defense in a position where x2 has to help guard 5. 1 makes a lob pass to 5 at the rim.

1 dribbles to the wing and passes to 5.
2 cuts to the rim.
3 cuts to the block.

5 dribble handoffs with 1.
4 cuts off of screens from 2 & 3.
