Here is a horns set play used against VCU in the NCAA Tournament. The guard popping out can either run a handoff on the weakside or hit the flare for a shot.

1 dribbles off of a ball-screen from 5.
1 passes to 2 popping out.

2 dribbles left to give 4 a handoff or passes to 1 flaring to the corner.
